New Student Enrollment (K – 12th)

All students must complete the enrollment process before they may attend the Ogden Community School District.  You may register online, at the Elementary School Office, the Middle School Office or at the High School Office.  Below, you will find information and links to necessary forms needed to complete the registration process.  Textbook rental and additional fees can be be paid in person in any building, by mail or on-line through the Infinite Campus Parent Portal.  Even though you will enter some enrollment information on-line, there are a few paper documents you must submit to your child's attendance center.

State law further requires that all students enrolling in Iowa schools furnish a certificate of immunization for diptheria, tetanus, pertussis (whooping cough), polio, measles, rubella, varicella and hepatitis B. Haemophilus influenza type b (Hib) vaccination and PCV is only required for children entering preschool. Beginning in 2013 all seventh graders are required to have a booster of Tdap immunization. The immunization requirement may be waived for religious reasons.State law requires that children entering kindergarten must be five years old on or before September 15. Children entering first grade must be six years old on or before that date. If a student has completed kindergarten in an accredited public or private school, in-state or out of state, he/she may be enrolled in first grade even if the child is not 6 by September 15. If a student has not completed kindergarten (e.g. moved mid-year) and was not 5 years of age by September 15, he/she cannot enroll in kindergarten in the State of Iowa. Federal law requires that children of military families be enrolled in the grade of the prior school regardless of age.

Parents or guardians must be present at the time of enrollment to furnish the child’s birth certificate or other proof of birth, immunization records and other educational available documents.
