New Employment Forms

The first five forms listed are required for new employees.  Existing employees may also use the below forms to make any changes to their personal tax withholdings, IPERS beneficiaries, or direct deposit banks. 

Any insurance changes must be done online.

Complete and return for federal tax withholding.
Centralized Employee Register (Iowa Tax Withholding)
Complete below the dotted line only and return for state tax withholding selections.
Direct Deposit Form
All employees are required to have their monthly paycheck direct deposited into the bank account of their choice. There is no option to receive a paper check.
Federal I9
Page 1 must be completed by the new employee. Page 2 is for the business office to complete. Please review the list of acceptable documents on page 3. You must provide 1 item from column A or 1 item from both columns B & C.
IPERS Beneficiary Form
Annual Use of School Vehicle Form
Required only for individuals that will be driving school vehicles.
